Class I indications for amiodarone therapy:
- chronic HF + ICD + symptomatic ventricular arrhythmias despite optimal pharmacotherapy and ICD reprogramming,
- chronic HF + ICD + recurrent ICD shocks despite optimal pharmacotherapy and ICD reprogramming.

Class IIb indication for amiodarone therapy:
- chronic HF + symptomatic sustained ventricular arrhythmias not requiring ICD.

Class III indication for amiodarone therapy:
- chronic HF + non-sustained ventricular arrhythmias.
