- chronic systolic HF + NYHA Class II - IV symptoms persisting despite optimal pharmacotherapy + atrial fibrillation adequate  ventricular rate control.

Class IIb indications for amiodarone therapy for maintain sinus rhythm:
- chronic systolic HF + NYHA Class II - IV symptoms + atrial fibrillation requiring direct current cardioversion,
- chronic systolic HF + NYHA Class II - IV symptoms + atrial fibrillation undergoing successful direct current cardioversion.
