Class IIb indications for amiodarone therapy for controlling the ventricular rate:
- chronic systolic HF + NYHA Class II - IV symptoms + persistent atrial fibrillation + inadequate  ventricular rate control during treatment with B-blocker,
- chronic systolic HF + NYHA Class II - IV symptoms + persistent atrial fibrillation + inadequate  ventricular rate control during treatment with digoxin,
- chronic systolic HF + NYHA Class II - IV symptoms + persistent atrial fibrillation + poor tolerance of B-blockers + poor tolerance of digoxin,
- chronic systolic HF + NYHA Class II - IV symptoms + permanent atrial fibrillation + inadequate  ventricular rate control during treatment with B-blocker,
- chronic systolic HF + NYHA Class II - IV symptoms + permanent atrial fibrillation + inadequate  ventricular rate control during treatment with digoxin,
- chronic systolic HF + NYHA Class II - IV symptoms + permanent atrial fibrillation + poor tolerance of B-blockers poor tolerance of digoxin.
